The Wake Forest University student named above has been furnished with the CrossPad1, serial number indicated above, for use during the academic term indicated above. The CrossPad remains the property of Wake Forest University. The student must return the CrossPad to Wake Forest University by the return date indicated above. If a student is not enrolled in Wake Forest University for any reason during the academic term indicated above, he/she must return the CrossPad to the University in good working condition upon separation from the University. Should a student temporarily enroll at another college or university, or study abroad in a program for which Wake Forest tuition is not paid, the student must return the CrossPad to the University. Students who do not return the CrossPad by the return date specified above, or upon separation from the University (if earlier than the return date indicated above), will be subject to charges equal to the full replacement cost of the CrossPad.
As with all other aspects of University life, there are rules and policies that govern usage of the CrossPad. In addition to the Student Honor Code, the University has a comprehensive Computer Usage policy to which all students must agree prior to using the University Network. The terms and conditions of the software on the CrossPad will also bind students. Copies of the policies and the software terms and conditions will be available to students in both distribution and training.
The University anticipates that students will use the CrossPad in all aspects of University life, but commercial use of the CrossPad violates the University's software agreements and is strictly prohibited.
Each student is responsible for the care of the CrossPad issued to him/her. The CrossPad must be kept in good working condition. The CrossPad will come equipped with batteries, ink cartridges, and a legal pad. The student will be responsible for furnishing replacement batteries, ink cartridges, and paper during the academic term, as necessary. If repairs are necessary, the CrossPad must be repaired through authorized Wake Forest repair programs. Other repair programs can only be used if the student obtains specific permission in writing from the Information Systems Support Center.
The CrossPad is not covered by the University's insurance policy. The Academic Computing Specialist named above, working in conjunction with the Information Systems Support Center, will provide support for all University-issued CrossPads during the academic term indicated above. Students are responsible, however, for replacement cost in the case of theft, negligence, or damage.2 A.T. Cross Company will make the determination on all negligence issues.
I have read the Program Agreement and agree to abide by the terms and conditions herein. Specifically, I acknowledge that the CrossPad remains the property of Wake Forest University. I understand that I will be responsible for furnishing batteries, ink cartridges, and paper for the CrossPad, if necessary, while I am using the CrossPad. If I am not enrolled in Wake Forest University for any reason, I will return the CrossPad to the Academic Computing Specialist named above in good working condition. I understand I will be subject to charges for the full replacement cost of the CrossPad if I fail to return it by the return date indicated above or upon separation from the University, should I leave the University during this academic term.
